Abstract Medical futility is commonly understood as treatment that would not provide for any meaningful benefit for the patient. While the medical facts will help to determine what is medically appropriate, it is often difficult for patients, families, surrogate decision-makers and healthcare providers to navigate these difficult situations. Often communication breaks down between those involved or reaches an impasse. Même si sa position est complexe, Aristote privilégie des interprétations intensionnelles plutôt qu'extensionnelles de sa logique syllogistique. Il n'utilise pas les fonctions de vérité, mais adopte une version de la sémantique qui accentue le rôle du langage naturel qui ouvre, pour ainsi dire, une fenêtre sur la réalité métaphysique. In this essay I attempt to elucidate the concept of an apology. I begin by considering the way in which apologizing is characterized by Erving Goffman; and I argue that his characterization does not suffice to distinguish the apology from many other speech acts. I then offer my own analysis, according to which (roughly) a speaker is apologizing to his hearer for something if and only if in saying what he does he is 1) expressing regret about it, 2) accepting (...) responsibility for it, 3) acknowledging it to constitute an offense to his hearer, 4) expressing regret about it as such, and 5) making a gesture of respect to his hearer as a person with a right to be spared such mistreatment. Context Physicians are regularly confronted with research that is funded or presented by industry. Objective To assess whether physicians discount for conflicts of interest when weighing evidence for prescribing a new drug. Design and setting Participants were presented with an abstract from a single clinical trial finding positive results for a fictitious new drug. Physicians were randomly assigned one version of a hypothetical scenario, which varied on conflict of interest: ‘presenter conflict’, ‘researcher conflict’ and ‘no conflict’. Participants 515 randomly selected (...) Fellows in the American College of Obstetricians and Gynecologists' Collaborative Ambulatory Research Network; 253 surveys (49%) were returned. Main object measures The self-reported likelihood that physicians would prescribe the new drug as a first-line therapy. Results Physicians do not significantly discount for conflicts of interest in their self-reported likelihood of prescribing the new drug after reading the single abstract and scenario. However, when asked explicitly to compare conflict and no conflict, 69% report that they would discount for researcher conflict and 57% report that they would discount for presenter conflict. When asked to guess how favourable the results of this study were towards the new drug, compared with the other trials published so far, their perceptions were not significantly influenced by conflict of interest information. Conclusion While physicians believe that they should discount the value of information from conflicted sources, they did not do so in the absence of a direct comparison between two studies. This brings into question the effectiveness of merely disclosing the funding sources of published studies. (shrink)

A modified version of the mere exposure effect paradigm was utilized in an implicit artificial grammar learning task in an attempt to develop a procedure that would be more sensitive in assesing nonconscious learning processes than the methods currently utilized within the field of implicit learning. Subjects were presented with stimuli generated from a finite-state artificial grammar and then had to either decide if novel items conformed to the rule structure of the grammar or rate the degree to which they (...) liked novel items. Because the latter of the task was more indirect of the two procedures, subjects′ availability to discriminate between well-formed and ill-formed items on this liking task was taken as a more sensitive piece of evidence of implicit learning, compared to performance on the rule conformity task. A subsequent test of subjects′ explicit knowledge of the rules of the grammar showed that subjects do have some conscious knowledge of the artificial grammar, but subjects making liking decisions exhibited significantly less conscious rule knowledge than subjects initially making rule-conformity judgements. The findings are discussed in relation to synergistic relationships among implicit perception, implicit memory, and implicit learning proceses. (shrink)
